Responsibilities
Provide patient-centered physical therapy in the home setting, helping patients improve mobility, strength, balance and everyday function. Evaluate patient needs, develop individualized treatment plans and monitor progress. Educate patients and caregivers on exercises, safe movement and strategies that support independence. Coordinate care with the clinical team and maintain accurate, timely documentation of visits and patient progress.
Qualifications, licenses & experience
The current job record lists three years of experience as a requirement.
