Responsibilities
Evaluate and treat patients in their homes. Develop and carry out individualized physical therapy plans of care, monitor progress, and educate patients and caregivers on safe mobility and home exercises. Coordinate care with the clinical team and document evaluations, treatment and progress in Homecare Homebase (HCHB).

Requirements & qualifications
A Massachusetts physical therapist license is required for this position. Previous home health experience and experience with Homecare Homebase (HCHB) are required.
