Responsibilities
Evaluate patients’ ability to perform everyday activities. Develop and carry out individualized occupational therapy plans, support safe daily routines and independence, and educate patients and caregivers on adaptive strategies. Coordinate with the care team and maintain accurate clinical documentation. Document care using Homecare Homebase (HCHB).

Requirements & qualifications
A Massachusetts occupational therapist license is required. Experience is preferred; the right candidate may be considered.
