Responsibilities
Provide skilled nursing assessment and care in the home setting. Follow the patient’s plan of care, monitor clinical needs, educate patients and caregivers, and coordinate with the clinical team. Coordinate with the care team and maintain accurate clinical documentation. Document care using Homecare Homebase (HCHB).

A Vermont registered nurse license is required. Experience is preferred; the right candidate may be considered.
