Responsibilities
Provide physical therapy treatment under the direction and supervision of a physical therapist. Support the established plan of care, monitor and report patient response, and educate patients and caregivers on prescribed exercises and safe mobility. Coordinate with the care team and maintain accurate clinical documentation. Document care using Wellsky.

Requirements & qualifications
A Massachusetts physical therapist assistant license is required. Home health experience required.
