01 · THE WORK
Responsibilities
Provide skilled nursing assessment and care. Follow the patient’s plan of care, monitor clinical needs, educate patients and caregivers, and coordinate with the clinical team. Coordinate with the care team and maintain accurate clinical documentation. Document care using Homecare Homebase (HCHB).
02 · YOUR EXPERIENCE
Requirements & qualifications
A New Jersey registered nurse license is required. Experience is preferred; the right candidate may be considered.
